What is causing this? Too much water?

Looks like mechanical damage, either something bumped into it or it was dropped head first. The growing point seems unaffected and it should be all right, but keep an eye on it so it doesn't spread.

I agree with jfabiao. It may just be mechanical injury. If the tissue underneath the lesion is soft and expands, it would indicate, that a microorganism (fungus, bacteria) has entered the plant at the areole. Then you should go ahead and excise the diseased tissue with a sterilized knife.
So, keep a close eye on your plant and take action as soon as you see a change for the worse.
