The new pad is growing etioliated(sp?). It happens when cacti do not receive adequate light. Put your opuntia somewhere it will receive full sun or the whole plant will start growing like that.

The high summer heat makes sense as the cause. I've had trouble and lost sulco's and other high altitude south American cacti in the past.
